The Ohio Hospital Care Assurance Program (HCAP) is an intensive financial assistance scholarship. This is for the infertile couples who want their treatment at the Cleveland Clinic.

Number and Amount of Grant Award

The exact amount is not publicized, but they offer adequate discounts on their services. The amount they discount you is based on your own wages and family income.

Award Coverage

Patients who opt for the award will get a special discount on their bills. The extent of which depends on the intensity of your current need. The award also covers all Medically Necessary Services and Emergency Care. Even if your insurance doesn’t cover it.

Application Fee and Eligibility

In order to apply, you must be a resident of Nevada, Florida (only specific counties), and Ohio. For couples whose annual family income is 250% of the Federal Poverty Guidelines (FPG), the HCAP offers a full discount. For couples having an annual income between 251% and 400% of the FPG, the discount is limited to what they would get if they had insurance.

People who have insurance can still apply if their insurance does not cover Medically Necessary Services charges.

The award will cover all your expenses related to fertility medication, treatment, and supplies. You can also use it to cover any Assisted Reproductive Technology (ART) procedure. This includes IVF, egg preservation, embryo preservation, and surrogacy, and gamete donations.

Application Fee and Eligibility

Only applicants who are legal residents or citizens of the US can apply for this gift fund. A diagnosis certificate by your health-care provider is also mandatory if all the information is true. All awards and gifts will be granted to your health-care provider and not to anyone individually. You must use the gift fund within 12 months of acquiring it.
